And that’s costing them business: clients have neither the time nor the money to deal with issues that never should have happened in the first place, so they “fix” the problem by hiring new lawyers.

The main safeguard against shoddy work: over-communicate with your clients. Always tell and retell clients what you are doing and why. And tell them once more, to play it safe.
